HEARING DETAILS
This summons is listed at [time, date and place to be inserted by the registry].
TYPE OF CLAIM
[Select type of claim from the list provided in section 6 of the Guide to preparing documents, available on the UCPR website at http://www.lawlink.nsw.gov.au/lawlink/spu/ll_ucpr.nsf/pages/ucpr_form_info - 3 or at any NSW court registry.]
[on separate page]
[Note: If the completed RELIEF CLAIMED will fit in the available space appearing after TYPE OF CLAIM on the first
page of this form, you may delete the page break, include the RELIEF CLAIMED on the first page and start this page with SIGNATURE.]
RELIEF CLAIMED
  1. [1] An extension to file an appeal as on July call over date I was institutionalised in psychiatric hospital.
  2. [2] His District Court Honour said "A Restraining Order is not appropriate in the circumstances,
however there is nothing else available." Her/His Honour Supreme Court must make a precedent
that allows a complaint or different type of order to be issued in grievance not involving restraining.
[3} Her/His Supreme Court Honour must set aside the Restraining Order and Break and Enter Charge
as Alexander Marcel Andre Sebastian Barker Bailiff as resident has got it through window of residence
he pays to live in spare room for 6 months as he does not have a spare key as his mother is psychotic.
[4} Her/His Supreme Court Honour must upon reading NSW Police Report of 7/12/85 make NSW Road &
Maritime Services financial liable for every cost incurred by all people injured and killed on poor cam bah.
Due to "superelevation in road being incorrect as road was designed for 30 years ago when speed limit was
lower as road was designed for 30 years ago when speed limit was lower." Call a NSW Judicial Commission.
[5] His Honour Chief Justice Terrence Higgins found "NOT GUILTY BY REASON OF MENTAL IMPAIRMENT."
As Mental Impairment is very severe brain damage occurred on 7/12/85 in a Motor Vehicle Accident
where "Super Elevation in the road is Incorrect as the road was designed 30 years ago when the speed
limit was lower", I would ask Her/His Honour make an order ensuring all criminal allegations since 7/12/85
be cleared from all criminal records in Australia after 7/12/85 and I would ask Her/His Honour clear all
Psychiatric Records in Australia since 7/12/85 and replaced with drstpehenallnutt.blogspot.com.au,
    professorsusiehayes.blogspot.com.au and drjohnmcmahon.blogspo.com.au.
